Today, healthcare systems increasingly involve multiple stakeholders including primary care physicians, specialists, hospitals, health plan representatives, pharmacy and other care-givers operating at different institutions.
While this movement has created a number of benefits for the patient, a new set of challenges have developed for practitioners: How can I access patient information that resides on another institution's network? How can I securely exchange patient information between different facilities? How can I be sure that referring physicians are examining my patients? How can I optimize my time and revenues?
Conservative estimates currently indicate that over 200 different EMR solutions are in use in North America today. In the United States alone, patient information resides in over 40,000 different databases spanning EMR, HIS and PACS used by over 375,000 physicians. Considering the wide range of proprietary software solutions that span today's healthcare system, a significant challenge exists for practitioners who need to collaborate on patient care across a community, where diagnostic information resides in different, isolated locations.

Schedule:
A user begins the collaboration process by scheduling an appointment, meeting, task, teleconference, or event.
Search:
After scheduling a collaboration session users can search all internal data stores for patient specific clinical information (EMR, PHR, LIS, CPOE, images, video, etc)
Configure/View:
After searching all patient specific data, results are provided for the clinician to select for visualization / collaboration purposes.
Collaborate:
After selecting the desired visual data, the user configures the clinical dashboard to be used during the collaborative decision making session and initiates the collaboration session by invoking the commercially available UC platform.
Act:
At the end of the session, users can schedule follow up actions. These actions include, but are not limited to: scheduling a patient appointment, requesting additional tests, documenting decisions in an EMR or via transcription services, email, etc.
The Aptus collaboration dashboard integrates with the underlying systems in bi-directional sense thus making the user experience a simple and consistent one throughout the various workflows that the clinicians handle.
